Notice of Privacy Practices

Psychotherapy Rights and Responsibilities:
Dr. Price views psychotherapy as a collaborative working relationship in which your active involvement and honest communication are crucial. In order for psychotherapy to be most successful, you will be working on your personal issues both during treatment visits and while at home. Together, you and Dr. Price will evaluate your goals and needs, and your level of distress. Part of the healing work will involve identifying your patterns of thinking, behaving, and relating that may impede your life goals, expression of emotions, and development of mutually satisfying relationships.

Psychotherapy can have benefits and risks. Since the treatment process involves discussing difficult aspects of your life, you may experience a variety of uncomfortable feelings. Psychotherapy has been shown to have benefits for people: it often leads to improved relationships, solutions to specific problems, and significant reductions in feelings of distress. Since people are complex, each and every patient will have a personal treatment experience unique to themselves. Dr. Price believes each patient sets the course for their own life, and is the one responsible for the decisions and life changes made. At times he may make suggestions and give advice, but always it is each patient who is in charge of what life choices are made and how they are implemented. Questions about Dr. Price’s procedures and his work with you are always welcome and should be discussed whenever they arise.

Dr. Price will support your decision to terminate psychotherapy at any time, for whatever reason. He does, however, request that you discuss your decision and reasons for termination at a regularly scheduled treatment visit. He will provide a referral to another qualified mental health professional without hesitation, at your request.

HIPPA:
You will find summary information about the Health Insurance Portability and Accountability Act (HIPAA) by clicking on the link below. HIPAA is a federal law that provides privacy protections and patient rights with regard to the use and disclosure of your Protected Health Information (PHI) used for the purpose of treatment, payment, and health care operations. HIPAA requires that Dr. Price obtain your signature acknowledging that he has provided you with this information.

Read it carefully. Print a copy and jot down any questions you might have so that you can discuss them at your first appointment. When you sign this document, it will represent an agreement between you and Dr. Price.
